WHO CAN VOTE BY MAIL
Any registered and qualified voter who is eligible to vote in an election may cast their ballot by mail.
Florida law requires a vote-by-mail ballot request to an address other than the elector’s address on file, must be submitted in WRITING and SIGNED by the voter. Please print, complete and return the form below to request a vote-by-mail ballot.

HOW TO REQUEST A VOTE-BY-MAIL BALLOT
A vote-by-mail ballot may be requested for a specific election or for all elections. One request is deemed sufficient to receive a vote-by-mail for all elections through the end of the calendar year of the next regularly scheduled General Election. The request can be made in person, by postal mail, by electronic mail or by telephone. Only the voter or designated member of his or her immediate family or legal guardian can request a vote-by-mail ballot for the voter. Immediate family means the voter's spouse or the parent, child, grandparent or sibling of the voter.
A request from the voter must include the following information:
- Voter's name and address
- Voter's date of birth
- Voter's Florida DL, Florida identification card number, or last four digits of SS#, whichever may be verified in the supervisor's records
- Voter's address where the ballot should be mailed
- The specific elections you are requesting
- Voter's signature (written requests only)
If the voter has designated an immediate family member or legal guardian to request a vote-by-mail ballot, that designee must provide the above information in addition to the following:
- The requester's name
- The requester's address
- The requester's Florida DL, identification card number or last four digits of SS#, if available
- The requester's relationship to the voter
- The requester's signature (written requests only)
OBTAINING YOUR VOTE-BY-MAIL BALLOT
All requested vote-by-mail ballots are sent 40 days prior to each election. Be sure the Elections Office has your correct mailing address. Vote-by-mail ballots are not forwarded by the US Postal Service. All requested vote-by-mail ballots for overseas military and overseas civilian voters are sent 45 days prior to each election.
VOTE-BY-MAIL REQUEST DEADLINE
Requests for a vote-by-mail ballot to be mailed to a voter must be received no later than 5:00 p.m. on the 10th day before the election by the supervisor. The supervisor shall mail vote-by-mail ballots to voters requesting ballots by such deadline no later than 8 days before the election.
VOTE-BY-MAIL PICK UP BY DESIGNEE
Pick up ballots are available up to nine (9) days before the day of an election. A qualified voter may designate, in writing, a person to pick up a vote-by-mail ballot for the voter. However, the person designated may not pick up more than two vote-by-mail ballots per election, other than the designee's own ballot, except that additional ballots may be picked up for members of the designee's immediate family. "Immediate family" means the designee's spouse or the parent, child, grandparent, grandchild, or sibling of the designee or of the designee's spouse. The designee must provide picture identification, the written authorization from the voter and complete an affidavit.
MARKING THE VOTE-BY-BALLOT
The voter must personally vote the ballot unless assistance is required due to blindness, disability or inability to read or write. The voter must sign/mark the return envelope. The vote-by-mail ballot will not be counted if the return envelope is not signed or the signature does not match the signature on file for the voter.

RETURNING YOUR VOTE-BY-MAIL BALLOT
You must mail, personally deliver or have someone else deliver your vote-by-mail ballot inside the completed mailing envelope to the Elections Office. A secure drop box is available at the Elections Office and Shelton's Public Library, available only during early voting hours.
Vote-by-mail ballots must be returned no later than 7:00 p.m. on Election Day. Vote-by-mail ballots cannot be turned in at the polling place on Election Day. If a voter has requested a vote-by-mail ballot and chooses to vote in person at the polls, the vote-by-mail ballot would then be cancelled by the Elections Office and the voter would be issued a new ballot.
Vote-by-mail ballots only require ONE (1) First Class or Forever stamp to return to the Calhoun County Supervisor of Elections Office.

Mail Ballots that are returned undeliverable to the Calhoun County Supervisor of Elections office because of an incorrect address, automatically cancels the voter’s future Mail Ballot request. The voter must make a new request providing the correct address.
